Print out and Fax in a Order Form for Either One of our Machines
Please Print and sign the completed form and fax it to our office. We will notify you by email that we recieved your faxed order form. We also will send you a email on the day your machine gets shipped and we will enclose the tracking number.

OUR FAX MACHINE IS ON FROM 8 a.m. to 10 p.m Eastern Standard Time Receive FAXES FROM ACROSS THE USA.
Our office will email you with a confirmation that we received the order fax during our office staff business hours.
Print out the order form, Fill is Completely out, Sign it ( signature is required ) and Fax it to us.

Below (at the bottom of this Page) you will find an Attachment that you will need to print out and then Fill out completely and sign and FAX it back to our office at 740-671-9754.
We ship within 10 business days once the signed faxed order form as been received.
You must complete the entire form giving us all the shipping information as well as Credit Card information and Credit Card Customers Card holder's name.
